Annette Lehoisky, 38, of Moreau, New York, was arrested by the Saratoga County Sheriff's Department after allegedly removing a patient's credit card number from a file at a family dentistry office in Wilton, New York, where she was employed. Lehoisky subsequently used the stolen account information to purchase a gift certificate from Noble Hardware in Moreau by telephone.

She was charged with fourth-degree grand larceny, a felony, and third-degree unlawful possession of personal identification information, a misdemeanor. Lehoisky was held at the county jail on $5,000 cash bail.
